Our foundation is transitioning to a NEW, online scholarship platform which will launch on December 1st, 2026.

Before applying, we recommend that you begin by preparing the following documents which will have to be uploaded and attached to your application. These will all need to be saved as .pdf files except for the photograph which must be a .jpg file.

a) Resume
b) Transcripts from your high school or post-secondary institutions
c) The FAFSA (Free Application for Student Aid) form, if applicable, showing your Student Aid Index number. 
d) Headshot photograph (this is for publicity purposes should you receive an award; reviewers have no access to it)

​​Applicants who are awarded a scholarship will receive a formal letter from the Community Foundation with details of how the scholarship will be paid and what the recipients need to do to receive the funds.  Notification of selection is made on a rolling basis, concluding by the middle of June.  Graduating seniors at Fort Atkinson High School receive their award letters at the awards ceremony held each spring.
